No, this is my second one of these ( the other one lost colour ,after a long while } & we have never had any problem with bits coming off or any breakage !

Yes - it's fraying around the edges after a few months use.

Is this mat machine washable?

Not that I know. I’ve never tried but a doesn’t look like it would machine wash safely. I have soaked mine in a bleach solution from time to time.
