What exactly comes in this setup?

Short tube with headgear and replaceable nose piece. Adjusters for head piece. I don’t think the long tube that connected to the machine was part of it. But order this kit 1st, just in case I’m wrong. 👍🏼Exact model I needed.

does this come in originally sealed packaging or repackaged in ziploc bags?

It's a 50/50 chance either way. I got it in a zip lock type bag once and in a sealed official box twice. It depends on who is offering it at the time. Either way, I washed them out before using them.

Is the head strap included?

No, you'd have to order that separately, and they are very inexpensive. If you order from Best Tech and pay almost $70 plus $4.99 shipping, then yes, it comes with the head strap.
