Can this keep shakes cold for more than 5 hours? (Does have milk in it)

Not for 5 hours - it’s pretty thin

Is it too small to fit for work lunch?

Not in my opinion. Of course, it would also depend on how much you put in it,the roomy enough big to hold mine lunch, water bottle, some fruits.I love this bag.

Can I place a flask that is 12 inches long diagonally?

Yes you can, this lunch box is very wide and opened on the inside. So far I have been able to fit 3-4 food containers, snacks and a drink or two inside and not have any issues.
