20-30 mins I think, I would definately recommend these

Can you heat in hot water rather than microwave?

I can't see why not. But I would think that the heat would be less strong and not lasting long. I use them as ice packs so I can't speak with any knowledge of heating them.

Can you refreeze them after one use?

Yes you can. I've refreezed mine several times over and they stay frozen for about 15 minutes. I just had mine on a rotation. Really handy for hot days too!
