I drove a friend's child who was wearing this vest. She could lean down and put her chin on her knees while wearing it. Is there a way to avoid this?

Were you using the top tether or just a lap/shoulder belt? If the vest was top-tethered I can't picture a kid leaning far down enough to be putting his/her chin to her knees. Some folks who might be better able to help you with this question are the certified child-passenger safety techs at the internet forum http://www.car-seat.org - they're very helpful and knowledgeable. Does this come with the back back, neck pillow and all the tethers? I can't find a lost in the description or pictures.

Does this come with a tether?

Sometimes the retailer at which you are currently will resell returns with parts missing since they do not know what to check for. All vests should come with a tether strap.
