Mine did. Case had more wear than I expected, but was a good buy.

Yes, there are a number of trophies in this game. Most range from silent kills to destroying army bases or performance in activities.

Is this worth getting??

Me personally I am a big fan of prototype so I would say yes. I have played this to the end. Easy storyline to keep up with and the game is not that hard to play. Depending on what kind of games you like but I think this will be one that most people will say it's worth getting.

Does this come with the cardboard sleeve / cover?

Mine comes in case with instructions but not cardboard
