How does this item get rated #5 in first aid work kits? And the orher drip torches are on a different list.

It must have gotten placed on the First Aid kit list by mistake. But as for the Drip Torch itself, I use mine every season. When the State Foresters came out to check my fire burn, they Tried to talk me out of it. They said the Red drip torch was Top Game tools. It's what they use.

Does it come with a spout?

If you mean what's shown in the picture, yes. This is a professional grade drip torch and I prefer it over a propane torch. The directions say to mix diesel with kerosene. On advice from fire fighter friends I use 1/4 gasoline and 3/4 diesel. Be careful not to drip it on your shoes!
