Nope, save yourself time and frustration
I had to send this item back, I had a CenturyLink technician stand my living room and troubleshoot this device and Amazon sold me a broken refurbished device! The device was a complete DOA after being supposedly refurbished and tested as working!

It does work but I still have buyer remorse
Received modem today it does work. After approximately 45 minutes on the phone with CenturyLink glad they were very patient. "Renewed item" in pretty beat-up up condition! Plenty of scratches and gouges on front and top.One of the four very short and sturdy legs was completely broke off and missing was not in the box. But on day one it is working.

Does not come with RJ11 phone cable
Product works. I am annoyed that it did not come with a phone cable. I wanted to take it out of the box and use it, but I couldn't.
