Q: How exactly do these hang from the wall? Does pre drilled hole mean you need a screw to match the color, or is the hanger hidden on the back?

It came with screws to screw into the wall. Here’s what mine look like hung up you do see the screws but they do match

Q: Would using a real candle burn the top of the holder or make it too hot?

I use real candles in my two sconces. Recommend using better quality pillar candles, as cheaper ones melt down around the bottom of the glass (I've been able to successfully remove cooled off wax.) inside top of glass gets sooty but glass cylinders can be carefully removed and washed clean in sudsy water. Any soot wipes off the metal holder.

Q: Do you have to use the glass pillar that comes with it or can you forgo that and use a candle in a glass jar and set it on the ledge

You could probably use a glass jar because the glass is a cylinder, open at either end, that sits on the round metal shelf if the sconces. I have two of these and love them!
