How many boxes will cover dining room 9/16ft and living room 12/18ft? Will it be any extra planks leftover from my measurements?

Your dining room requires 2 2/3 d boxes ( so 3 boxes) and your living room you need exactly 4 boxes. Total 7 boxes and you should have a few planks extra. Thanks!

is any preparation necessary for the floor? I will be ripping out carpet. Some indicated the tiles didn't stick. Is extra adhesive necessary?

Generally extra adhesive is not necessary. In some cases if the subfloor is not perfectly clean or if you are trying to install over an extremely smooth surfaces it may be required.
