Is the actual diameter of the brush 22mm OR does it fit inside 22mm tubing ?

The brush fits into 22mm tubing quite easily. The diameter of the brush itself is roughly 25mm. Remember it has soft, flexible bristles, so don't worry too much about the diameter of the brush itself. The brushes are attached to slender, flexible wire handles and do a great job at cleaning CPAP tubing and other parts. Hope this helped.

How do you remove the brush from the tube without breaking the tubing??

Must get a hold on one end of brush and push towards the other end and pinch the end a little and let loose the other . This will work the piece to the end and keep this up repeating procedures till you can remove the piece by your fingers.

Is it safe for heated tubes?

It's safe b/c the bristles are so soft and short, they won't even touch the inside of the tube. Tube came in plastic and smooshed the bristles flat losing it's round shape. Replacing my tube, but first returning this.
