The directions say to spit it out after 15 seconds. How can it be spit out if it was sprayed onto the back of the throat?

There is literally no way to spit out something that has been directly sprayed on the back of the throat. That part of the directions is there for 2 reasons. First, it comes from when people used to gargle liquid instead of spraying it. Second, it's a way for Chloraseptic to cover themselves (as they should) in case people swallow the spray (as they will) and get an upset stomach because of it (rare). 1 spray, swallow, let it sit as long as you can (15 seconds hopefully), then swallow.

Is it also anti septic? [kills germs?]

No, it does not kill germs. It is an oral anesthetic for sore throat.

How long does the numbing last?

Between "did this even spray?" and "not friken long enough." Joking aside it doesn't last very long at all.. minute.. maybe
