Q: Is the bridge already on the violin?

Mine was. And the strings, which are all that's holding the bridge in place, were tensioned and (approximately) in tune.

Q: Is this an intermediate violin?

My wife says yes and that she feels it is a great violin but that it would need a better bow.

Q: I have had some bad luck with tuning keys, wearing out and flopping about after a few times tuning it, is this model relibale?

I have had mine two years with no tuning peg problems. When I tune with the Pegs I push inward strongly. But I rarely need to use the pegs , mostly use the fine tuning screws.
