This phones fits the Guardian RFID Spartan which is based on the Bluebird EF500R. That is like two Note 8's fit together. That is a very thick phone with large corners for fall protection. I use this device at work. I am very happy with this product.

Q: How do attach it to a cell phone samsung galaxy note 9 in otterbox case ?

Galaxy note 9 is a Big phone in general it was tight for my iPhone which was smaller

Q: Does this holder have a built in belt loop or just hook and loop mount?

yes, it only has a built in belt loop. I had to add the hook myself.
