Does this mat leave scratches on the top of club head when hitting or swinging?

Not that I have seen. With any mat, you could see some mat reside on the bottom of the club just from the heat of the club, but not scratches

Product is very good. Allows for practicing multiple shots when driving range is not available. Helps to develop a repeatable swing.

Can you place a tee anywhere on the mat?

No it is just as it looks in the picture with one tee holder location.

How tall is it in total? Says 16mm turf, but with the base, how tall is it from the base to the top of the turf?

The height of the base and turf together is just under 1 inch. However, I hit it on my concrete floor and it is no issue...and I am a low single digit handicap. Mostly use it in combo with the net to keep swing mechanics during winter, and in using the driver or 3w off of a tee it is no difference.
