What is the maximum weight capacity for this chair?

The balance ball is extremely sturdy..it will hold a 180 easy. So I do believery it could carry a weight of 200 with no problem

does the chair base swivel? not rock back and forth, but move left tor right / right to left like a traditional office chair?

Yes, it is very smooth. swivel. It turns 360 degrees. I love the chair. I sit in it everyday at work, sometimes 11 hours. I have zero complaints. You have to add air every now and then, depending how firm you want it. I like that too, because I can fix it just the way I want it.
