Does this product require one to drink water to use it?

HiccAway is most effective when both the vagus and phrenic nerves are stimulated. The vagus nerve is stimulated by the pressure differential, and the phrenic nerve is stimulated when we swallow. In other words, HiccAway is most effective when a small amount of water is swallowed. Other customers have found relief without water, but that generally is not ideal.

I get stubborn cases sometimes that wake me up in the middle of the night, does this help with stubborn cases?

Does not prevent - should cure the incident. You might try a triangle pad under your pillow to ease the tubing in your neck.

When you're sucking on the straw, should you be inhaling or exhaling?

You should be inhaling. You want the unit to go from virtually empty, to full of water, and then swallow, repeating 2-3 times. The goal is to lower the diaphragm through powerful suction, and swallow, in close proximity.
