Can you pull the chain once to turn it on/off or do you have to cycle through all the different brightness settings?

Yes you can pull the chain once and it will turn the light off. You don't have to cycle through all the other brightnesses. My wife loves it!!!!!

Does it come with a bulb?

yes, our floor lamp comes with a 3 color temperature bulb.

Can you leave the light in one setting? Every time I turn it on it changes

It should turn on to the lowest brightness, but that is the setting we leave our lamp on. Is your lamp connected to a switched outlet? If so, try plugging it in to a regular outlet.
