Is this hand dryer ADA approved?

I bought one a few months ago and it is already broken...

I purchased 3. I have not had any problems what so ever. Not sure how it would get broken. Maybe someone damaged the sensor or you got a lemon :/ Again I have 3 and the first 1 purchased well over 5 months the others have been in use for a few months now and still no problems. Sorry to hear, you should probably reach out to the company that makes it.

I installed 8 of these so far, but one is ghost operating; occasionally it will start by itself, and run for a few seconds. Any ideas how to remedy?

It is likely that something is triggering the sensor. Check to make sure the sensor area is clean. Also note that the sensor picks up infrared signals and reflections if it is within proximity. If you find that there is an issue with the hand dryer itself, please contact the seller directly by going to your orders and clicking on "contact seller." They will take your information and send out a new one right away.

what is the decibel level on this unit? It will be installed into a small bathroom so it can't be too loud

It is approximately 70 decibel at a distance of 3 feet. It is a high-speed dryer and moves air very quickly. Most high speed dryers have sound levels at 70 decibel and above. the advantage of a high-speed unit like this is that it dries hands very quickly. Unfortunately, this does create a certain noise level. There are some slower velocity dryers that would be less noisy, but they also dry hands more slowly.
