I think you could use it by just placing your heels closer to the edge of the heel rest. There's enough surface area on the heel rest that your feet should be secure throughout the movement. By having your heels in the proper location, the force of the weight would be applied across the top of your foot and not forcing your toes to absorb the force.

Q: What about foot/shoe size? Is it adjustable for those with small feet?

It is not adjustable but it fits most shoe sizes. For example, my teen son has small feet with shoe size 6 1/2 and he uses it comfortably without issue. The padding really hugs your foot so there are no need for adjustments. I have 11 1/2 shoe size and it also fits me well.

Q: What is the best seating position? Should I sit low or high?

Having your thighs at 90 degrees to your calves is probably ideal, but I do not suggest sitting lower than that. Higher is better.

Q: How would you fit bigger plates, being so close to the ground?

Unless you are the Incredible Hulk, you probably won’t need bigger plates. My 25 pound plates fit on the sleeves just fine with adequate clearance.
