Does swtch need backing box

Yes it does, but just a slim 'architrave' box - I used this surface mounted one: https://www.amazon.co.uk/gp/product/B00ENGK2ZK/ref=oh_aui_detailpage_o00_s00?ie=UTF8&psc=1 (which has doubled in price since I bought mine); alternatively you could use a galvanized box sunk into wall/plaster.

Can this be configured / wired as 'push to break'?

The switch has a common terminal and both push to make and push to break terminals. Looks like a two way switch from the back.

will this work as a bell push?

Yes it is. I use it as an external garage door opener which is on the same principle as a bell push i.e. normally open switch, and press and the bell will ring..Good quality will last indefinitely, better than the cheap bell pushes..
