Like other people that have commented, I had lost a of my hearing on my right ear. Could feel pressure and fluid inside. Nasal spray didn't help. Pinch nose and blow didn't help (not appropriate in my case anyway). I tried the balloon produce (ottovent) both by blowing the balloon and the reverse and they didn't make a difference. This product is similar in operation to letting the balloon deflate into your nose, but the difference is that it can handle the back pressure (unlike the balloon). So don't expect a fast stream of air (you probably wouldn't want that anyway), but it's more a case of being able to deliver it even when you're trying exhaling during the swallow maneuvre. I saw immediate improvement after the first 4-5 times I used it (during a 20min period). The next morning I could hear much better but could still feel a bit of pressure in my ear. I tried it again this evening, again 4-5 times, and now my ear feels completely normal. Huge relief after weeks of not being able to hear properly! Yes, it's a bit pricey for what it does, but it's a small price to pay to get one of your senses back! This did nothing for my negative middle ear pressure or did alleviate any of my symptoms at all. I don't know if my device is faulty or something because the stream of air is incredibly weak. Does absolutely nothing.
