Will it work as an additional rail at the bottom of a hospital bed?

Yes. It would work wherever you place it. It slides under the mattress.

Does this work with an adjustable frame bed?

It works great with my adjustable bed. I have a California King Bed. I bought a second one for the other side of the bed, because I like to switch sides. Only thing bad I can say, but it's minor thing, the bag that comes with it snaps on. I have one snap that will come undone. I just have to be careful not to put anything a little heavy in the bag.
