In my case, 2 stacked in each shoe works well for me. I have supronation so I put the thick side on the outside. If you have promation you'll put the thick side on the inside. The biggest surprise is how much better they make my lower back feel. I bought a bunch of these for most all my footwear...

I ordered these insoles almost two weeks ago and I am pretty impressed with the way they felt when I put them inside my sneakers. They’re comfortable and fit just right. However putting them together proved more like a great challenge, such as peeling off the coverings to “stack” the wedges correctly was no walk in the park. Otherwise I felt the difference when I put the inserts inside my sneakers, meaning they alleviate some pain in my heels. I look forward to wearing these wedge inserts to get my feet aligned. Therefore I give it 8/10

I went to the podiatrist, and he fitted me with foot orthotics to counter pronation. That was about $600 for a couple of uncomfortably rigid inserts.These may not be custom orthotics that are specially fitted at a doctors office. BUT they actually feel comfortable enough to wear for long periods AND they work!I am very happy with the results of wearing these inserts and I am very happy to save hundreds of dollars.
