Are these reusable? Or do you need to replace them?

I wash mine with antibacterial soap, then leave them in a solution of water and miltons steriliser overnight. I have used mine for months befor replacing them. They have really helped me to manage my prolapse. I couldn't be without them.

How do you know what size to get?

I am afraid it was trial and error there is no one size fits all you can get a suitable pessary fitted by gp or hospital I have a prolapse good luck. These are good for stress incontinance

do you insert them like a tampon and how long do you leave them in situ

Yes you do use like tampon, but I am sure the instructions say do not leave any more than a day. they are quite good but you have to boil them for about 5 minutes after use that can be a pain. would be great if they were disposable.
