Will this work with an Otterbox defender case on? If not what will?

Using a case with your phone shouldn't interfere with it's wireless charging capabilities.Wireless charging works best with slim, flexible cases that are no thicker than 5mm so that the transmitter and receiver can connect easily. The cases must also not be made of metal. Wireless charging worked with the Otterbox Defender case on the Samsung S6, but unfortunately we don't have that case for the G4 to test it with.

Will this allow me to charge my lg ls991 with an itian a9 qi wireless charger transmitter pad?

Yes it should make your lg ls991 compatible with any Qi Wireless charging pad.

Does it work with the LG G4 US991 (us cellular, unlocked for gsm) model?

Yes, I installed it to the leather back of a G4 with Verizon (carrier shouldn't matter), and it works as advertised - using an OEM Samsung wireless Qi charger.
