Will this case be suitable for an xs wearing an otterbox commuter?

This case is custom-made to fit iPhone 11/XR with a medium case on it. The iPhone Xs has a 0.3" smaller screen than the XR, so it will fit with a Commuter case on it, it could just be a little loose on the top. Monsoon does not make this model for the Xs, but I think this particular model for Xr will serve your purpose for the Xs.

How reliable is the clip?

The belt clip is very reliable, at least it has been so far. The metal belt clip has a lip that surrounds the belt and keeps the holster from sliding off of the belt. That lip also makes it difficult to manually slide the holster off of the belt when you want to take it off. As long as you don't have too wide of a belt, the holster is very secure. A belt wider than 1 3/4" would be too wide. The holster would still clip onto your belt, but it would slide off easily. Hope that helps.
