That means that an XL in this size would correlate to an S or XS in women's sizing. I'm about 5' 1" and 100 pounds. Personally, I like my underwear tight, so I purchased the large. It fits comfortably snug. I'm sure I would've been able to wear the XL also but I stand by the large.As to effectiveness, these are not supposed to be standalone period protection. They're not like Thinx period panties or anything. They are simply backup. They would probably hold as much as a pantiliner. Another thing is that the leakproof layer stops around mid crotch, so this may not be the best for front bleeders. I bleed in the middle or back so it is fine for me. I love that it is super thin. It looks and feels just like regular underwear and it is very comfortable. I highly recommend it! ### ⭐ This product does not work unfortunately *by J***N on January 24, 2018* I immediately bought these for my dtr to use after starting her cycle and being nervous about leaking through. After opening the package I couldn't imagine that these would offer any protection but we decided to try them out. After only a short time of using these with a pad in place, she leaked through her clothing. She is very upset and this was not a good experience. I read lots of good reviews and that is why we went with this product. Her normal underwear were more protective than these were.
