Q: does it have a screw mechanism for micro adjustment or is it simply a push button lock.

It is continuously adjustable because the entire bar is a screw drive. Works well.

Q: Does it come with instructions?

Yes I believe it does, however, mine came with just one slide out bar when the picture showed 2-3. Just wedge it between the slide out and the motorhome wall.
