Is there a place to put name and phone number on back of ticket if customer does not need to be present to win?

Yes but the numbers are smaller than other tickets we have had. If your population is under 40' they will probably be fine.

I understand every ticket has a 6 digit number but eliminating the first 3 numbers, will they all start at 0 or 1? Meaning, 987001 or 453001?

Yes all of our tickets are 2000 sequenced numbers starting at 001. None of our tickets in the same color have the same number. There are some tickets out there that all have the same numbers on them, so maybe just an FYI.
