Can this equipment be used indoors, or is it for outside use only?

The stove says on it "NOT FOR HOUSEHOLD USE", and the instruction manual contains 4 different warnings about using it inside (reproduced below). When butane burns it normally produces harmless carbon dioxide; however, if there isn't sufficient oxygen available (such as when in an poorly ventilated space) then it will form toxic carbon monoxide instead. If you do decide to ignore the warnings and use it indoors, then make sure that the room is well-ventilated, and make sure that you have a carbon monoxide detector.

The interior of the manual has a "DANGER" section that is mostly about explosions and fire hazards, but contains these 2 warnings about using it indoors:

WARNING: Provide ample ventilation when using this appliance. This appliance consumes oxygen. To avoid endangering your life, do not use this appliance in unventilated structures. Provide additional ventilation for any additional burning products or additional occupants.

This appliance is for outdoor use as a camp stove or indoor use in commercial restaurants or both. It is not for household use. It should never be used in an unventilated structure such as a tent, camper, vehicle or any other enclosed area as the use of oxygen or the bulid [sic] up of carbon monoxide could endanger your life.

Appoximately how many hours of cooking can i get with 1 fuel can using it on the gas one gs-3000 portable gas stove? at approx. a med-high setting?

We took this bad boy camping for 2 weeks and went through 1 1/2 cans of fuel (cooking nearly every night). I would stock up on cans however, as some states dont have these readily available. This stove is great! Love the sturdy carry case too!

where is this stove made? how long does 1 butane canister last, for example, typical 3 day camping trip, cooking 3 meals a day? durable?

I would think china.  We use it for cooking outside and a canister of butane lasts a long time.  But, that's just me and we're typically cooking one dish and not a dinner.  You may use more.  It's cheap and often sold in 3 and 4pks.  If you're cooking for 4 people 3 x day =12 meals, I would guess two might be as much as you'd need.  But I suggest you buy a 4 pack and see  how much you use.  If you're feeling scientific you could make an approximation by weighting the canister, cooking a typical meal and weighing the canister again and seeing  how much  you used.

Question 4

Is anyone know what kind of hose adapter I will need to have this works with a 20 lb propane tank instead of use with 8oz. cans?

I don't know the answer, but I have doubts about the safety for two reasons. One is that the burner is designed for butane, not propane. That difference is an issue as seen for those who want to use a burner using propane vs. natural gas.  The pressure can be different, and they do burn at different temp. so I assuming the butane gas grill is designed for a certain temp. and pressure from a butane canister.
