Does the plastic easily break?

I have had the brace for six months. 4 basketball games per week with brace on. No sign of plastic breaking.

Does it work with flat foot and navicular bone accessory?

Yes I have flat feet and works well. Best brace I have had to play basketball

What are the two extra pieces of velcro for? The directions are awful and dont explain where they go?

Not sure what you mean by "extra" pieces...two straps criss cross around the foot & get stuck on the ankle. Then there's the straps that go around the ankle to fasten it.
