How do i measure where to secure this? The directions are not very good.

Put cover on hot tub. Cross bar on near seam. Place attachment to hot tub with 1 screw. Have another person help. Lift cover to make sure both sides are even and off the ground. Watch a few you tube videos

Is this strong enough to hold a very heavy water-logged cover?

possibly, if you hold the cover while lifting it. ours bent an arm when our kids lifted it and let it fall back. Broke both plastic mounting brackets also.

How much room do I need on the side, and once it goes up behind the hot tub?

I have about 1 foot behind on the sides about 1'-4" I just lift it up and get in then fish lifting once in the tub very easy to use and install.
